Before we can develop a site for you, we will need you to send us some information about your company and any preferences you have about the look and function of your website. Please consider the Questionnaire carefully and send us your responses via one of the following methods:

• Email: We strongly encourage this method, as it allows us to develop your site more quickly. Include your Questionnaire response in the body of the message, or as a plain text or MS Word formatted attachment. Send email to djw[at]djwizard[dot]com, with the subject heading "Design Questionnaire".

2. Company Information
   Describe your company. Are you wholesale or retail? What products and services do you offer? What qualities does your company have that sets it apart from your competition? Does your company have a specialty, and if so what is it?

3. Design Services
   Do you require any design services such as website design or logo design? If you are not using DJW design services for your website, we will need source files from your designer to review before we can estimate your project.

4. Jewel Pavilion
   Which of our Jewel Pavilion packages are you interested in? If you are not certain, describe the inventory you would like to sell online and we can help you select which packages suit your business.

5. Existing Content
   Is there any existing content or any existing promotional graphics that you wish to appear on your site? For example, company logos, print ads, old website pages, charts, information of any kind. This information should be provided to DJW during the estimate process.

6. Desired Content
   What kind of additional information do you want to have on your website? This includes things like Education sections, FAQ pages, promotional graphics, etc. We need a specific page count and graphics count for your estimate.

7. Hosting and Domain Name
   Do you need Hosting services or domain name registration for your website? Would you like to host via a third party hosting service or via a local server in your office? ***Jewel Pavilion requires a specific hosting environment - please contact us before purchasing hosting on your own.*** You may purchase your domain name at any time, although we recommend you purchase it as soon as you decide on your website name.

8. Inventory
   For diamond or gemstone inventories, we will need to know what the source of the inventory is. This includes DJWizard and Rapnet or Polygon inventories. For jewelry inventories, you can start entering jewelry data after we install Jewel Pavilion to your hosting account, or receive a one time jewelry upload from the DJWizard jewelry module. Jewelry photos will be uploaded by DJW as they are received from you and processed.

10. Your Ideas
    We want to hear about what you want to see from your website. If you have any preferences in color, style, layout, anything, please note them at the end of your questionnaire. If there are websites you find particularly attractive or functional, please include the URLs of those websites as well. We are looking to match what you envision for your website, so the more information you provide, the faster we can achieve it.

Some of items on the Questionnaire ask you to send a photograph or picture. We prefer that pictures be digital, that is, already scanned or otherwise in the computer. Attatch digital images to your Questionnaire email, or put the images on a zip disk or CD-ROM and mail to our conventional address. Disks must be in a PC/Windows format.
If it is not feasible to send your picture digitally, you may mail us printed images. Be sure that the picture quality is good enough for scanning. DO NOT send photocopies, faxes or low-resolution inkjet printouts. If you are unsure about an image, call us and ask.
